**Job Title: PRN CNA/HHA (Certified Nursing Assistant/Home Health Aide)**
**Job Description:**
We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) / Home Health Aide (HHA) to join our healthcare team on a PRN (as-needed) basis. **Key Responsibilities:**
- Assist patients with daily living activities such as bathing, dressing, grooming, and eating.
- Monitor and document patients' health, including vital signs and changes in condition.
- Provide support in ambulation and mobility, ensuring safety and comfort.
- Assist with medication reminders under the supervision of licensed nursing staff.
- Help maintain a clean and orderly environment for patients.
- Build positive and supportive relationships with patients and their families.
- Follow established care plans and communicate any necessary adjustments to the clinical team.
- Maintain confidentiality and adhere to HIPAA regulations.
**Qualifications:**
- Current certification as a Nursing Assistant (CNA) or Home Health Aide (HHA) in the state of employment.
- CPR and First Aid certification is preferred.
- Proven experience in a similar healthcare setting is advantageous.
- Excellent communication and interpersonal skills.
- Empathetic and patient-centered approach to care.
- Ability to work independently and as part of a team in a dynamic environment.
- Flexibility to work on a PRN basis, including some evenings, weekends, and holidays.
